Referral Requirement/Maintaining Network Benefits 2024-2025

Student Health Insurance Policy Year: August 16th through August 15th

Note: Campus Health referrals expire at the end of each policy year. Referrals must be renewed every August 16th for ongoing care. 

  • To renew a mental health referral, please call Counseling & Psych Services (CAPS) at 520-621-3334.
  • To renew a medical referral, please message your Campus Health provider through PatientLink to see if a renewal is possible. If an appointment is required to renew your referral, please call the Campus Health appointment line at 520-621-9202. 

Fall Coverage:  August 16th - December 31st  (provides 4.5 months of coverage)

Spring Coverage:  January 1st - August 15th (provides 7.5 months of coverage, includes summer months)

Summer Session I & II:  June 1st - August 15th (provides 2.5 months of coverage)


Maintaining Network Benefits

All three state university student health centers (UA, ASU, and NAU) serve as primary care providers for the enrolled members under their plan. To schedule an appointment with UA Campus Health, please call 520-621-9202 for medical appointments or 520-621-3334 for CAPS appointments. 

  • Referral is Required: for services received within 25 miles of the UA Main Campus (85721).  If the community provider refers you for additional care, UA Campus Health must authorize that referral before being seen.
     
  • Referral is Not Required: for services received beyond 25 miles (based on zip code where services are received) from the UA Main Campus (85721); treatment of an emergency medical condition at an Emergency Room; participating Urgent Care Facilities (all follow-up treatment received within 25 miles of the UA Main Campus must obtain a referral from UA Campus Health); Obstetrics and Gynecological Treatment; Routine Eye Exams; Preventive/Routine Services (services considered preventive according to the Affordable Care Act and/or services rendered not to diagnose or treat an Accident or Sickness); Injury to Sound, Natural Teeth; and Impacted Wisdom teeth.

To find participating medical and behavioral health providers, visit www.uhcsr.com/arizona and select Find Providers. You may also log on to your UHCSR "My Account" to search for providers and explore care options. It is the member’s responsibility to ensure they are seeing UnitedHealthcare participating providers. List is subject to change.

  • HealthiestYou – Students insured by UHCSR have access to free Mental Health and General Medicine telehealth services through the HealthiestYou website or mobile app. Login to your UHCSR member account to view your benefits or visit www.uhcsr.com/hycounseling for more information. 
  • UHCSR Student Assist – Students can receive confidential assistance with personal issues and crises. Services include mediation, legal and financial help, self care, and 24/7 crisis support. The toll-free Student Assist phone number is available in your UHCSR member account under My Benefits (then click on "Additional Benefits"). 
  • How to Read an EOB – Learn how to read your UHCSR Explanation on Benefits (EOB). EOBs are documents from the insurance carrier that outline how services were covered and if there is a patient responsibility. EOBs are available through your UHCSR member account under My Claims
  • How to Submit a Claim – Did you visit an out-of-network provider or pick up a prescription from the pharmacy without providing your insurance information? You can submit medical and prescription claims to UHCSR through your member account. 

ID cards are available online through your UHCSR member account or through the UHCSR mobile app. You may request that a physical card be mailed to you by logging into your account or calling UHCSR Customer Service at 1-866-654-7445.

Once enrolled, you will be automatically re-enrolled and billed the appropriate premium through the UA Bursar's office in future semesters (each fall and spring) within 3 business days of showing registration units, providing you meet eligibility. This also applies to students who have or had a Graduate Assistantship. If you wish to cancel coverage, you must do so during the published open enrollment.

The Campus Health Insurance Office sends out open enrollment notices each Fall and Spring semester through broadcast email. In compliance with the UA email policy, all open enrollment notices and information regarding the health care options are sent to the student's official UA email address (@arizona.edu). Students are expected to check their email on a frequent and consistent basis in order to stay current with university-related communications.

  • You will also receive important plan notifications by email from the insurance carrier, UnitedHealthcare StudentResources (UHCSR).
