Your Electronic Health Record

An electronic health record (EHR) is created for you upon receipt of your immunizations and/or at your first appointment with a provider at Campus Health. The information contained in the health record is confidential and protected by state and federal laws. The information in your health record belongs to you and you have a right to request a copy of your health record.

To obtain a copy of your EHR:

  1. Complete an Authorization for Release of Information form
  2. Attach a copy or upload a picture of your photo ID with the form
  3. Submit the form with a copy of your ID through your PatientLink Portal* or fax it to (520) 626-4301.
  4. Records requests over 10 pages will incur a $5.00 copying fee. If the fee applies to your request, you will be contacted by Medical Records staff for payment.
  5. Allow up to 14 business days for processing your request.

*If you are not currently a UA student or do not have access to PatientLink, please complete an Authorization for Release of Information Form and then upload the authorization.

Submitting Prior Records To Your Campus Health Provider

If you would like a Campus Health provider to have an outside record relating to your upcoming appointment at Campus Health, upload your medical record to our secure system. To expedite your submission, we ask that you write "outside record for appointment" in the comment box.

Campus Health Services For Minors

In order to be seen at the clinic, a minor (under 18) must have the Consent To Treat A Minor Form on file with Campus Health.  The form must be signed by the minor's parent or legal guardian. To submit the form, upload it through your PatientLink Portal or fax it to 520-626-4301.

Medical Records FAQs

If you are an active student, you can log into your PatientLink portal and print your immunization record. After logging in, select 'Immunizations' and click the print link in the upper right corner.

ACCESS PATIENTLINK

If you are not an active student, you will need to complete an Authorization for Release of Information form and fax it to (520) 626-4301.

In accordance with Arizona law, medical records are retained for a minimum of 10 years.

  1. Complete an Authorization for Release of Information form
  2. Attach a copy or upload a picture of your photo ID with the form
  3. Active students can submit the form with a copy of their ID through their PatientLink portal or fax it to (520) 626-4301.
    *If you are not an active student, you can submit your form to us by fax: (520)-626-4301.
  4. Some records requests will incur copying fees. You will be notified in advance if the fee applies to your request.

If you are fully vaccinated, please upload your vaccine document to the secure, HIPAA-compliant system.

Counseling and Psych (CAPS) records are not released through the Medical Records Department. You will need to complete a CAPS Authorization for Release of Information and submit your request to the CAPS office. For more information on obtaining CAPS records please call the CAPS office at (520) 621-3334.

Under Arizona and federal  law, you may be charged a reasonable fee for the production of your medical records. Record requests greater than 10 pages incur a $5.00 fee. If you make payment in person, you can Bursar the fee. Payments can also be made over the phone via credit/debit card.

Copying fees are waived for medical records provided from CHS directly to another healthcare provider for continuity of care.

The Medical Records department processes most record requests within 14 business days. However, state and federal law allow up to 30 days for processing record requests. Processing times are always longest around graduation.
