Medical Documentation from the Health Care Provider
To withdraw from the current term or complete a retroactive withdrawal request for medical reasons, follow the steps below:
- Download the Medical Provider form
- Fill in your student information at the top of the form
- Provide the form to your healthcare provider
- Your healthcare provider completes their portion of the form and returns it to you
- Upload the completed form with your withdrawal request
Note: This form is the only accepted medical documentation for medical withdrawals.
