If you wish to access health care services through Campus Health, you will be required to pay the Health portion of the UA Health & Recreation (H&R) Fee.
- Arizona Online, Southern AZ & Distance Learners: To access services at Campus Health under the Student Discounted Rate or with your Commercial Health Insurance Plan, please submit the health fee request form below.
- Students who have enrolled in the Student Health Insurance Plan or CampusCare do not need to submit this request. The health fee will automatically apply to your Bursar account.
- Arizona Online students are not eligible for the UA Student Health Insurance Plan. However, Arizona Online Students who had the health plan in Spring/Summer 2022 will continue to be eligible until graduation, change in eligibility status, or if they cancel coverage during the Open Enrollment period.
