Your Electronic Health Record

An electronic health record (EHR) is created for you when you submit your immunization record.  Your EHR belongs to you, is confidential, and is protected by state and federal laws. You have the right to access your EHR through PatientLink or you can request a copy of your health record.

 

Request A Copy of Your EHR:

Request Records to be Sent Elsewhere via PatientLink Portal:

  1. Log into your PatientLink Portal
  • Click on Messages
  • Click on New Message
  • Select I'd like To Request A Release of Medical Records
  • Fill out electronic release form

Request Records to be Sent Elsewhere OR to be sent to Campus Health Service:

  1. Complete an Authorization for Release of Information form
  2. Upload form or fax it to 520-626-4301 (you must include a copy of your photo ID).

Record requests over 10 pages will incur a $6.50 fee.

Allow up to 14 business days for processing.

For CAPS Records

  1. Fill out the CAPS Authorization for Release of Information
  2. Upload form or fax it to 520-626-4301 (you must include a copy of your photo ID).

If you are an active student, you can log into your PatientLink portal and print your immunization record. After logging in, select 'Immunizations' and click the print link in the upper right corner.

ACCESS PATIENTLINK

If you are not an active student, you will need to complete an Authorization for Release of Information form and fax it to (520) 626-4301.

In accordance with Arizona law, medical records are retained for a minimum of 6 years.

  1. Complete an Authorization for Release of Information form
  2. Attach a copy or upload a picture of your photo ID with the form
  3. Active students can submit the form with a copy of their ID through their PatientLink portal or fax it to (520) 626-4301.
    *If you are not an active student, you can submit your form to us by fax: (520)-626-4301.
  4. Some records requests will incur copying fees. You will be notified in advance if the fee applies to your request.

In accordance with Arizona law, medical records are retained for a minimum of 6 years.

Under Arizona and federal law, you may be charged a reasonable fee for the production of your medical records. Record requests greater than 10 pages incur a $6.50 fee. If you make payment in person, you can Bursar the fee. Payments can also be made over the phone via credit/debit card.

Copying fees are waived for medical records provided from CHS directly to another healthcare provider for continuity of care.

The Medical Records department processes most record requests within 14 business days. However, state and federal law allow up to 30 days for processing record requests. Processing times are always longest around graduation.
