The Medical Records department maintains your original medical record which is the property of Campus Health Services, however, the information in it belongs to you.
Prior to receiving copies of your records or authorizing records to be disclosed, you will be asked to complete an Authorization For Release of Information Form and provide a picture ID. Once we receive the written authorization the processing time can take up to 14 business days.
How do I obtain my health records?
A copy of your health record is available after an Authorization For Release of Information form has been submitted along with a picture ID. In person records are released at the medical records window located on the 2nd floor of Campus Health. Copies of x-rays are released in the x-ray department on the 1st floor. Mental Health records are released in the CAPS department on the 3rd floor after a CAPS release is signed. Please call (520) 621-4068 for questions relating to release of your health records.
After completing the form, you can upload to our secure site, fax or hand carry to our office. Options for receiving the record are in person, mail, fax and email*.

Outside Medical Records?
If you have an appointment with our providers and would like them to have an outside record relating to your appointment upload the file for fastest processing. Due to peak immunization submissions, we ask you write in the comment box "outside record for appointment". This will help us expedite your record.
Currently enrolled students may print a copy of their immunization record by clicking on PatientLink and following the instructions. Alternatively, immunization records can be obtained via fax, mail or in person at the Campus Health Service. These records are not kept indefinitely and will be purged.
