Medical Records

Medical Records department maintains your original medical record which is the property of Campus Health Services, however, the information in it belongs to you.

Prior to receiving copies of your records or authorizing records to be disclosed you will be asked to complete an Authorization for Release of Information form and provide a picture ID. You may be charged a copying fee for your records; you will be notified if any fees will be incurred. To reduce copy charges you may request that only recent information or specific documentation be printed. Processing times for non-medical emergencies can take up to 10 business days.

How do I obtain my health records?

A copy of your health record is available after a release (pdf) consent form has been signed along with a picture ID. In person records are released at the medical records window located on the 2nd floor of Campus Health. Copies of x-rays are released in the x-ray department on the 1st floor. Mental Health records are released in the CAPS department on the 3rd floor after a CAPS release is signed. There is no charge for records sent to another doctor for continued care. Please call (520) 621-4068 for questions relating to release of your health records.

After completing the form, you can upload to our secure site, fax or hand carry. Options for receiving the record are in person, mail, fax and email*.
